Introduction: The selective RPE treatment (SRT) is a new method, which targets retinal diseases associated with disorders in the retinal pigment epithelium (RPE). By applying a train of μs laser pulses, it is possible to selectively damage RPE cells while sparing the adjacent photoreceptors and the neural retina. Due to the ophthalmoscopic invisibility of the RPE effects we investigated an optoacoustic (OA) on-line dosimetry system to monitor RPE damage non-invasively. Material and Methods: For in vitro experiments porcine RPE was irradiated with a Nd:YLF laser pulse train (527nm, 1.7μs, 5-40μJ, 30 pulses, 100 Hz). Pressure waves (optoacoustic transients) generated at the RPE were measured with a piezoelectric transducer. The RPE cell damage was visualised by fluorescence microscopy by means of the vitality stain CalceinAM. During 27 patient treatments (527nm, 1.7μs, 50-150μJ, 30 pulses, 100 Hz) the optoacoustic signals were measured with an ultrasonic transducer embedded in the contact lens. The RPE leakage was visualized by fluorescein and ICG angiography. Results: In vitro: Below the RPE cell damage threshold, the optoacoustic transients from each single pulse are almost similar. With RPE damage, fluctuations of the individual transients are observed during the pulse train. These fluctuations can be explained by statistical irregular microbubble formation around the strong light absorbing melanosomes inside the RPE cells, which occur after the temperature exceeds the vaporization threshold. The transient microbubbles probably lead to RPE cell disruption. An optoacoustic value (OA-value) calculated from the fluctuations was defined in order to assess RPE damage. Patient treatment: If optoacoustic pulse-to-pulse fluctuations were measured, RPE leakage was observed in fluorescein and ICG angiography. In 96% of the irradiated areas, RPE-leakage in fluorescein angiography and OA-value correlated. The stronger the optoacoustic pulse-to-pulse fluctuations, thus the higher the OA-value, the more intense angiographic leakage was observed in ICG-angiography. Conclusion: A non-invasive optoacoustic on-line dosimetry control to monitor RPE damage during SRT was developed. In order to avoid invasive angiography, it is currently evaluated in a multicenter clinical SRT study.

Background: Protein denaturation in the fs-ns time regime is of fundamental interest for high precision applications in laser tissue interaction. Conjugates of colloidal gold coupled to proteins are presented as a model system for investigating ultrafast protein denaturation. It is expected that irradiation of such conjugates in tissue using pico- up to nanosecond laser pulses could result in effects with a spatial confinement in the regime of single macromolecules up to organelles. Materials and Methods: Experiments were done with bovine intestinal alkaline phosphatase (aP) coupled to 15 nm colloidal Gold. This complex was irradiated at 527 nm/ 532 nm with a variable number of pico- and nanosecond pulses. The radiant exposure per pulse was varied from 2 to 50 mJ/cm² in the case of the picosecond pulses and 10 to 500 mJ/cm² in the case of the nanosecond pulses. Denaturation was detected as a loss of protein function with the help of the uorescence substrate 4MUP. Results and Discussion: Irradiation did result in a steady decrease of the aP activity with increasing radiant exposures and increasing number of pulses. Inactivations up to 80% using 35 ps pulses at 527 nm with 50 mJ/cm² and a complete inactivation induced by 16 ns pulses at 450 mJ/cm² are discussed. The induced temperature in the particles and the surrounding water was calculated using Mie's formulas for the absorption of the nanometer gold particles and an analytical solution of the equations for heat diffusion. The calculated temperatures suggest that picosecond pulses heat a molecular scaled area whereas nanosecond pulses could be used for targeting larger cellular compartiments. It is difficult to identify one of the possible damage mechanisms, i.e. thermal denaturation or formation of micro bubbles, from the dependance of the inactivation on pulse energy and number of applied pulses. Therefore experiments are needed to further elucidate the damage mechanisms. The observed inactivation dependencies on applied energy and radiant power can not be explained with one or two photon photochemistry. In conclusion, denaturing proteins irreversibly via nanoabsorbers using pico-/ nanosecond laser pulses is possible. The expected confinement of the heat to the nanoabsorbers suggests that denaturation of proteins with nanometer precision could be possible with this approach. However, the mechanism of protein inactivation, which is part of present investigations, is crucial for the precision of such nanoeffects.

This complex was irradiated at 527 nm wavelength and 35 ps pulse width with a varying number of pulses ranging form one up to 10⁴. The radiant exposure per pulse was varied form 2 mJ/cm² to 50 mJ/cm². Denaturation was detected as a loss of protein function with the help of the fluorescence substrate 4MUP. Results and discussion: Irradiation did result in a steady decrease of the aP activity with increasing radiant exposures and increasing number of pulses. A maximal inactivation of 80% was reached with 10⁴ pulses and 50 mJ/cm² per pulse. The temperature in the particles and the surrounding water was calculated using Mie's formulas for the absorption of the nanometer gold particles and ana analytical solution of the equations for heat diffusion. With 50 mJ/cm², the particles are heated above the melting point of gold. Since the temperature calculations strongly depend on changes in the state of matter of the particles and water, a very sophisticated thermal model is necessary to calculate exact temperatures. It is difficult to identify one of the possible mechanisms, thermal denaturation, photochemical denaturation or formation of micro bubbles from the dependance of the inactivation on pulse energy and number of applied pulses. Therefore, experiments are needed to further elucidate the damage mechanisms. In conclusion, denaturing proteins irreversibly via nanoabsorbers using picosecond laser pulses is possible. The confinement of the heat to the nanoabsorbers when irradiating with picosecond pulses suggests that the denaturation of proteins could be possible with nanometer precision in cells with this approach. Optical Coherence Tomography (OCT) is a new, non-invasive diagnostic technique for high resolution optical 3D imaging, which was developed and applied to several different biological materials during the lasi; five years [1, 2, 3]. A unique application ofthis technique is the microscopical cross-sectional imaging ofpostenor structures ofthe eye which are not accessable with other high resolution techniques in-vivo neither with x-ray-imaging nor with high frequency ultrasound scanning. The superior spatial resolution on the order ofabout lOtm laterally and axially, the high signal-to-noise ratio ofmore than 100 db and the fast acquisition-time of one second for a two dimensional scan provides a technique for cross-sectional in-vivo-momtoring ofintraocular structures and therefore the possibility to study the time course of anatomical and pathological developments in the eye. The acute morphological changes of ocular structures and their biological healing response after shortterm impacts such as high-intensity laser exposures are ofparticular interest for the understanding of the mechanisms responsible for therapeutic laser-application in ophthal-mology as well as for laser injury to the eye. A correlation between cross-sectional OCT-images and structural findings using classical histopathological techniques facilitates a better interpretation ofthe characteristic patterns seen in OCTimages and defines the sensitivity ofthe OCT-technique to image morphological details. On the other hand preparational artefacts not avoidable in all histological procedures can be identified and analyzed by comparing histological micrographs with OCT-images of exactly the same structure. First results of an experimental study where retinal effects were produced in monkey eyes using laser pulses from 200 ms to 130 fs in duration are presented in this article. The applied energies from 5tJ to 50 mJ were able to induce the whole spectrum of biological effects possible in the eye, ranging from intraretinal microruptures to extensive thermal denaturation and massive preretinal hemorrhages [4, 5, 6].
