Vessel-based lung lobe partitioning in ultra-short time echo proton MRI for regional ventilation assessment

This paper addresses the problem of lung lobe partitioning in ultra-short time echo (UTE) MRI acquisitions, which are recently used for lung ventilation assessment with MRI spirometry. Because of the low image contrast, which does not enable the lung fissures display, the developed approach relies only on the vascular structures which still can be segmented from these images. The vascular network is segmented in lobes in order to generate reference clusters used for lung space partitioning. A point cloud representing the unstructured points of the vascular medial axis is partitioned in five lobes exploiting the PointNet++ framework. The PointNet++ model is trained on data extracted from CT acquisitions and labeled using the airway and vascular trees connectivity. The airway tree lobes will define the lung lobar regions, which are propagated on the vessel structure to achieve the complete vascular labeling. A separate model is trained for the right and left lungs in order to alleviate for limited input point cloud size imposed by the model architecture and reach a high precision in classification. The trained model is applied to UTE-MRI data to generate, for a given subject, a point cloud reference that will be used for vascular lobes clustering, which will be then exploited for lung space partitioning in lobes. The approach was quantitatively evaluated on 10 CT volumes from LUNA16 dataset and qualitatively tested on additional 25 CT and 15 UTE-MRI datasets. The analysis of CT data results shows pertinent lung partitioning with respect to the lung fissures, even if a precise fissure localization is not achieved. Such result is however expected, since no information related to the lung fissure is exploited in our method because this would not be applicable to UTE-MRI data. Nevertheless, the proposed partitioning respects the vascular lobes and, to the best of our knowledge, is novel for lung MRI sequences making it possible the regional investigation of ventilation parameters in MRI spirometry. The method can be further on extended for lung fissure matching in CT data by integrating new constraints related to fissure detection.

The COVID-19 infection, a current worldwide health concern, manifests as an alveolar-interstitial pneumonia with unknown long-term evolution. It is also associated with vascular dysfunction and shows a vascular remodeling with a changed balance between small- and large-caliber vessels. In this study, we question the existence of residual vascular alteration in post-acute sequelae of COVID-19 (PASC) by investigating possible associations between vascular remodeling biomarkers extracted from CT and functional, radiological and morphological parameters. The used vascular biomarkers concern the blood volume ratio of vessels with cross-section area inferior to 5 mm2 versus vessels of crosssection area inferior to 50 mm2 (BV5/BV50), an index of local peripheral vascular density and a peripheral composite vascular remodeling index, both measured in the antero-postero-lateral lung periphery (excluding mediastinal region). As a functional parameter, diffusing capacity of the lung for carbon monoxide (DLCO) is a measure depending on the vascular perfusion and the amount of interstitial thickening, a decreased DLCO value suggesting altered vascular perfusion. Imaging biomarkers can be extracted from the analysis of perfusion lung scintigraphy or CT scan. Some of them are included in our study. Radiological features include CT attenuation as a measure of persistence of ground glass opacity and development of changes suggestive to look for fibrosis, such as reticulations. As additional morphological parameter, lung deformation observed between inspiration/expiration maneuvers may be suggestive of the presence of reticulations inducing lung stiffness and breathing deficiency. The investigation of associations between vascular remodeling biomarkers obtained from CT and the above functional, radiological and morphological parameters revealed moderate to strong correlations highlighting the ability to capture the persistence of vascular alterations in PASC in relation with the development of fibrotic patterns, which is a promising direction for future research.

Hereditary hemorrhagic telangiectasia (HHT) is an autosomic dominant disorder, which is characterized by the development of multiple arterio-venous malformations in the skin, mucous membranes, and/or visceral organs. Pulmonary Arterio-Venous Malformation (PAVM) is an abnormal connection where feeding arteries shunt directly into draining veins with no intervening capillary bed. This condition may lead to paradoxical embolism and hemorrhagic complications. PAVMs patients should systematically be screened as the spontaneous complication rate is high, reaching almost 50%. Chest enhanced contrast CT scanner is the reference screening and follow-up examination. When performed by experienced operators as the prime treatment, percutaneous embolization of PAVMs is a safe, efficient and sustained therapy. The accuracy of PAVM detection and quantification of its progression over time is the key of embolotherapy success. In this paper, we propose an automatic method for PAVM detection and quantification relying on a modeling of vessel deformation, i.e. local caliber increase, based on mathematical morphology. The pulmonary field and vessels are first segmented using geodesic operators. The vessel caliber is estimated by means of a granulometric measure and the local caliber increase is detected by using a geodesic operator, the h-maxdomes. The detection sensitivity can be tuned up according to the choice of the h value which models the irregularity of the vessel caliber along its axis and the PAVM selection is performed according to a clinical criterion of >3 mm diameter of the feeding artery of the PAVM. The developed method was tested on a 20 patient dataset. A sensitivity study allowed choosing the irregularity parameter to maximize the true positive ratio reaching 85.4% in average. A specific false positive reduction procedure targeting the vessel trunks of the arterio-venous tree near mediastinum allows a precision increase from 13% to 67% with an average number of 1.15 false positives per scan.

The analysis of pulmonary vessels provides better insights into the lung physio-pathology and offers the basis for a functional investigation of the respiratory system. In order to be performed in clinical routine, such analysis has to be compatible with the general protocol for thorax imaging based on multi-slice CT (MSCT), which does not involve the use of contrast agent for vessels enhancement. Despite the fact that a visual assessment of the pulmonary vascular tree is facilitated by the natural contrast existing between vessels and lung parenchyma, a quantitative analysis becomes quickly tedious due to the high spatial density and subdivision complexity of these anatomical structures. In this paper, we develop an automated 3D approach for the segmentation of the pulmonary vessels in MSCT allowing further quantification facilities for the lung function. The proposed approach combines mathematical morphology and discrete geometry operators in order to reach distal small caliber blood vessels and to preserve the border with the wall of the bronchial tree which features identical intensity values. In this respect, the pulmonary field is first roughly segmented using thresholding, and the trachea and the main bronchi removed. The lung shape is then regularized by morphological alternate filtering and the high opacities (vessels, bronchi, and other eventual pathologic features) selected. After the attenuation of the bronchus wall for large and medium airways, the set of vessel candidates are obtained by morphological grayscale reconstruction and binarization. The residual bronchus wall components are then removed by means of a geometrical shape filtering which includes skeletonization and cylindrical shape estimation. The morphology of the reconstructed pulmonary vessels can be visually investigated with volume rendering, by associating a specific color code with the local vessel caliber. The complement set of the vascular tree among the high intensity structures in the lung may also inform on lung pathological conditions (inflammation, interstitial disease, nodular patterns,...). The results obtained on normal and pathological subjects or in inspiration vs. expiration, are presented and discussed.

Follow-up studies of bronchial wall remodeling in asthmatics based on multi-detector computed tomography (MDCT) imaging is the emerging modality for therapy assessment. While providing statistically significant quantification of global variation before/after treatment, the employed cross-section-area estimation techniques cannot guarantee the absolute accuracy of point-by-point estimation. Such uncertainty comes from the impossibility to define an accurate cross-sectional plane of a bronchus at locations where the notion of central axis is questionable. In order to overcome such limitation, this paper develops an original automated volumetric approach for bronchial wall quantification involving a successive model-based 3D reconstruction of the inner and outer bronchial wall surfaces. The inner surface is segmented by means of strong 3D morphological filtering and model-fitting. An optimal geometrico-topological model is generated by using a restricted Delaunay triangulation approach. The model is then dynamically deformed in the surface normal direction, under the constraint of local energy minimization acting at each evolving vertex. The energy potentials oppose a mesh-derived elastic component combining topological and geometric features in order to preserve shape regularity, and an expansion potential exploiting image characteristics. The deformation process both adapts the mesh resolution and handles topology changes and auto-collisions. The developed 3D modeling stabilizes the deformation at the level of the outer surface of the bronchial wall and provides robustness with respect to bronchus-blood vessel contacts, where image data is irrelevant. The accuracy of the volumetric segmentation approach was evaluated with respect to 3D mathematically-simulated phantoms of bronchial subdivisions. Comparisons with recent 2D techniques, carried out on simulated and real MDCT data showed similar performances in cross-section wall area quantification. The benefit of using volumetric versus cross-section area quantification is finally argued in the context of bronchial reactivity and wall remodeling follow-up.
